Board-certified family nurse practitioner licensed in Montana, Texas, and Alaska. Emergency-trained. WorldLink-certified in advanced bio-identical hormone replacement. Founder of an independent practice built so Montana patients do not have to settle for a seven-minute visit.

MSN, APRN, FNP-BC, a-BHRT
Licensed NP — Montana, Texas, and Alaska
Sarah Cruse is a board-certified Family Nurse Practitioner and the founder of 406 Health Consulting in Billings. She is board certified through the ANCC and currently certified in ATLS, CALS, PALS, ACLS, and NRP. She holds active NP licensure in Montana, Texas, and Alaska. With full practice authority under Montana law, she operates an independent, self-pay practice focused on family medicine and advanced integrative wellness — concierge-level care tailored to rural and underserved communities.
A graduate of the University of Mary (FNP, 2012), she brings more than 15 years of clinical experience spanning emergency medicine, acute care, family practice, and outpatient settings. She currently contracts with Wilderness Medical Staffing to cover rural and Critical Access Hospital emergency departments, hospitalist service, and step-down ICU — the same high-acuity work that trained the rapid assessment now sitting underneath a slower, preventive, patient-centered practice.
After years of providing medical and trauma care under fluorescent lighting and beige walls, she built 406 Health Consulting as the opposite: a warm room, enough time, and a plan that treats quality of life as a clinical outcome.
Emergency rigor. Optimization science.
What distinguishes Sarah in the hormone and wellness space is the pairing of emergency-level clinical judgment with evidence-based optimization medicine. She is a certified Advanced Bio-identical Hormone Replacement Therapy (a-BHRT) provider through WorldLink Medical. Her WorldLink profile notes more than 15 years of emergency provider experience incorporating quality-of-life care via BHRT. That emergency and inpatient work is current, not archival: rural and CAH departments, hospitalist coverage, and step-down ICU through Wilderness Medical Staffing.
That combination matters when hormone care sits next to comorbidities, medications, and the kind of complexity franchise protocols are not written for. She also holds advanced certifications in aesthetic procedures, so regenerative and cosmetic work can live inside the same plan rather than a separate spa menu.

A self-pay practice, by design.
We do not accept or bill medical insurance. Transparent pricing is provided before visits. The model exists so time, protocols, and patient experience are not dictated by a billing department — and so rural patients can know the cost of concierge-level care before they walk in.
The philosophy is whole-person care “across the lifespan during times of complex vulnerability” — perimenopause, high-performer optimization, acute needs, telemedicine follow-up — with the same standard that defined the emergency years.
