406 Health Consulting uses and discloses protected health information to treat you, to obtain payment for services, and to operate the practice. We may also use or disclose information when required or permitted by law.

Website consult requests and new patient intakes are emailed to the practice at sarahcruse@proton.me. Proton Mail encrypts mail in that inbox. Those web forms are not a substitute for the Spruce or Tebra patient portal. Do not send emergency information through the website.

You have the right to request restrictions, confidential communications, access to and amendment of your records, and an accounting of certain disclosures. You may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

42 CFR Part 2

406 Health Consulting is a small, independent NP-led practice focused on family medicine, integrative wellness, hormone optimization, weight management, and aesthetics. It is not a designated Part 2 program under 42 CFR § 2.11: it does not hold itself out as providing substance use disorder treatment, nor is SUD diagnosis or treatment the primary function of any staff or unit.
